Job summary

Total Care Therapy LLC in Ohio is seeking a compassionate Physical Therapist to join our growing team in Waterville. You will evaluate, develop, and implement individualized treatment plans to improve mobility, restore function, reduce pain, and maintain independence for residents in assisted living and related settings.

The role offers full-time scheduling, Monday-Friday with weekends available but not required, and compensation ranging from $88,000 to $94,000 annually, with potential per-visit

Qualifications

• Current State Physical Therapist license.
• CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
