Job summary

AltaMed is seeking a capable Physical Therapist to join our PACE program in California. You will evaluate mobility dysfunction, develop individualized plans, and work to restore function, mobility, and overall health for frail elderly and disabled patients.

You must hold a California PT license and have at least 1 year of geriatric experience, with a current BLS certification. Bilingual English/Spanish/Cantonese/Mandarin is preferred, supporting diverse communities.

Qualifications

• Current California Physical Therapist license.
• At least 1 year of geriatric and disabled population experience.
• Valid BLS certification or higher as per AHA/Red Cross guidelines.
• Bilingual preferred: EN/ES/CN/MN.

Responsibilities

• Evaluate mobility dysfunction and create a plan of care tailored to individual goals.
• Help restore function, improve mobility, and relieve pain.
• Support prevention of permanent physical disabilities through targeted therapy.
