Job summary

AltaMed Health Services Corp. seeks a Physical Therapist–PACE to restore function, improve mobility, relieve pain, and prevent disability. You will assess mobility limitations and develop an individualized plan of care aligned with patient goals for functional independence.

The role requires California licensure, at least 1 year with frail elderly or disabled populations, and a valid BLS or higher. Bilingual in English, Spanish, Cantonese, or Mandarin is preferred.

Qualifications

• California-licensed Physical Therapist required.
• Minimum 1 year experience with frail elderly (geriatric) and disabled populations.
• Valid BLS certification or higher per AHA/Red Cross guidelines.
• Bilingual English/Spanish/Cantonese/Mandarin preferred.

Responsibilities

• Evaluate mobility dysfunction and formulate a plan of care tailored to individual goals.
• Provide interventions to restore, maintain, and promote functional independence and mobility.
