Qualifications

• Must have one (1) year of experience with a frail or elderly population.
• Active State of Michigan licensure is required.
• Education must include a Physical Therapy degree at any level.

Responsibilities

• Coordinate rehabilitation services in conjunction with the Interdisciplinary Team.
• Document all care provided to participants in a timely manner.
• Perform initial and periodic assessments related to participant care.

Skills

Physical ability to instruct and demonstrate exercises

Experience with frail or elderly population

Ability to order/issue Durable Medical Equipment

Education

Physical Therapy degree (Bachelor, Master, or Doctoral)

Active State of Michigan licensure
