Total Care Therapy is hiring a compassionate Physical Therapist to join our growing team in Southeast Michigan. You will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, and maintain independence in assisted living and independent living communities.

Qualifications

• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.
• Current State Physical Therapist (PT) License.
• Current CPR/BLS Certification.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.
