Job summary

Continuum Therapy Partners in Jasper, FL is seeking a licensed Physical Therapist to join our therapist-led team and help expand access to high-quality care.

You will evaluate, design, and implement individualized treatment plans, document progress, educate patients, and collaborate with other disciplines. This is a part-time daytime role at Suwannee Valley Nursing Center in Jasper.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:
