Job summary

Continuum Therapy Partners in Wisconsin seeks a licensed Physical Therapist for a part-time daytime role at Stevens Point Health Services and affiliated facilities. The candidate should hold a Doctor of Physical Therapy (DPT) degree, a current state PT license, and NPTE eligibility, with 1 year of PT experience preferred.

You will evaluate, develop, and implement individualized treatment plans, document progress, educate patients on home programs, and collaborate with other disciplines.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomesli>
