Continuum Therapy Partners, LLC in West Chester Township, OH is seeking a qualified Physical Therapist to join our therapist-led network. You will use a DPT to assess patients, develop personalized treatment plans, and collaborate with a multidisciplinary team to support patient recovery.

Qualifications

• Must hold a Doctor of Physical Therapy degree (DPT).
• Current, unencumbered state Physical Therapy License; NPTE must be passed.
• Minimum 1 year of PT experience preferred.

• Documents patient progress and adjusts plans of care based on outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
