Job summary

Live Well Homecare is seeking a licensed Physical Therapist to provide personalized care to older adults in private homes and senior living communities in Naples, FL. The role offers a flexible schedule with telehealth options to support aging in place.

You will evaluate, design, and implement individualized treatment plans, document in EMR, and collaborate with an interdisciplinary team. The position supports in-person and virtual sessions with a focus on mobility and independence.

Qualifications

• Valid PT license in the state.
• Degree from an accredited Physical Therapy program (DPT or equivalent).
• Proficient with EMR systems and general computer literacy.
• Current CPR certification.
• Ability to meet physical demands (lifting and assisting patients).
