Seacrest Behavioral Health is seeking a Physical Therapist to join a home health care team, helping patients recover at home. The role includes assessing mobility and function, setting goals, and delivering therapy per physician orders. You will educate patients, discharge planning, and ensure compliance with regulations.

Responsibilities extend to supervising PTA/HHA staff where applicable and performing OASIS assessments as allowed. A valid PT license and CPR certification are required.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification.
