Job summary

Palmshomecare is expanding its Home Health team and is seeking a skilled Physical Therapist (PT) to provide in-home services, improving mobility and independence for patients recovering from illness, injury, or surgery in Gainesville, FL.

You will perform initial evaluations, develop individualized treatment plans, lead therapeutic exercises and manual therapy, educate patients and caregivers, and maintain accurate documentation while following home health and Medicare guidelines.

Qualifications

• Must hold a current Physical Therapist license.
• Graduate from an accredited PT program.
• 1–2 years of experience in Home Health is preferred.
• Strong communication skills are essential.
• Ability to work independently.
• Valid CPR certification is required.
• Valid driver’s license and reliable transportation.
• Knowledge of OASIS.
• Experience with EMR systems such as Kinnser.
