Job summary

Palms Home Care in the Town of Florida, NY is seeking a skilled Home Health Physical Therapist to join our team. You will provide individualized PT services in patients' homes, perform assessments, develop treatment plans, and help clients regain mobility and independence through evidence-based therapies.

The ideal candidate holds a current PT license, graduate of an accredited PT program, and 1–2 years in home health.

Qualifications

• Current Physical Therapist license
• Graduate of accredited PT program
• 1–2 years of experience preferred in Home Health
• Strong communication skills
• Ability to work independently
• Possesses and maintains current CPR certification.
• Valid driver’s license and reliable transportation
• Knowledge of OASIS
• Experience with EMR systems such as Kinnser
