Job summary

Hireology is seeking a dedicated Physical Therapist (PT) to join our in-house rehab team. You will evaluate patients, develop individualized plans, and administer PT treatments to improve mobility and quality of life.

Join a collaborative interdisciplinary team, ensure documentation compliance, and participate in ongoing education and meetings as designated by leadership. Texas licensure is required and SNF experience is preferred.

Qualifications

• Active and valid Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Attend required meetings as designated by the Director of Rehab.
