Job summary

Hireology is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will provide person-centered care to help residents improve mobility, strength, and overall quality of life in a Skilled Nursing Facility setting.

Responsibilities include evaluating patients, designing tailored treatment plans, administering therapies, and coordinating with an interdisciplinary team. Licensure in Texas and prior SNF experience are preferred; strong communication is essential.

Qualifications

• Active and valid Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to assess physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using various therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer PT treatments to relieve pain, restore function, and promote healing.
• Ensure treatments align with the care plan and state/federal regulations.
• Monitor patient progress and adjust approaches with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team for high-quality care.
• Complete documentation in compliance with policies and regulations.
• Attend required meetings as designated by rehab leadership.
