Job summary

Hireology seeks a Per Diem Physical Therapist (PRN) to join our in-house rehab team serving residents with person-centered care in Texas. You will assess, plan, and implement PT interventions to improve function and reduce pain, working within established plans of care.

Texas-licensed PT with SNF/long-term care exposure is preferred. This position offers flexible scheduling and in-house rehab support within a collaborative IDT environment.

Qualifications

• SNF/Long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Familiarity with Net Health EHR preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain and improve or restore function.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment and modify as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team to ensure quality patient care.
• Ensure documentation is completed thoroughly and timely and complies with regulations.
• Attend required meetings as designated by the Director of Rehab.
