Job summary

Hireology is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will provide person-centered care, evaluate mobility, design treatment plans, and collaborate with an interdisciplinary team to enhance residents' quality of life.

The role requires active Texas PT license, SNF or long-term care experience preferred, and familiarity with Net Health. We offer competitive compensation and benefits, flexible scheduling, and opportunities for professional growth.

Qualifications

• Active Texas Physical Therapist license in good standing.
• SNF or long-term care experience preferred.
• Familiarity with Net Health documentation system.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Monitor patient response to treatment and modify approaches with the interdisciplinary team.
• Complete documentation thoroughly and timely in compliance with policies and regulations.
• Attend required meetings as designated by the Director of Rehab
