Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is seeking a qualified Physical Therapist for full-time or part-time roles. The position offers a supportive environment, competitive wages, and opportunities for professional growth within a long-term care setting.

Responsibilities include comprehensive PT evaluations, treatment planning, documentation, and collaboration with an interdisciplinary team.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy that is recognized by the APTA.
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Document findings on the standardized evaluation form, accurately and adhering to guidelines
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
• Document patient progress weekly and update treatment plans as needed.
• Attend facility and rehab meetings with supervisor or regional manager.
• Supervise junior staff or students; co-sign notes as required.
