Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license/registration in the state (as applicable).

Responsibilities

• Provide comprehensive PT evaluations based on MD orders.
• Develop and document treatment plans with goals, frequency, duration and modalities.
• Obtain MD orders for evaluations, treatments, re-certifications and discharges.
• Deliver comprehensive treatment and adjust plans per patient progress and precautions.
• Document daily participation and progress; update treatment plans and discharge summaries as needed.
• Co-sign notes and supervise junior staff or students; participate in staff education as needed.
