Hertford Rehabilitation And Healthcare Center in Hertford, NC is seeking a Physical Therapist (PT) for full-time or part-time positions. A $10K relocation bonus is offered. You will evaluate and treat residents, develop care plans, and document patient progress within established guidelines.

Formación

• Graduate of an accredited university with a degree in Physical Therapy (B.S., DPT, MA or MS).
• Current license/registration in North Carolina as applicable.

Responsabilidades

• Provide comprehensive PT evaluations based on physician orders.
• Develop and implement treatment plans with goals, frequency, duration, and modalities.
• Obtain orders for evaluations, treatments, and discharges.
• Document progress and update treatment plans and discharge summaries.
• Collaborate with the interdisciplinary team and participate in facility meetings.
• Supervise staff or students as needed and co-sign notes.
• Maintain professional relationships with patients, families, and staff.
