YAD Management is seeking a Physical Therapist for Hertford Rehabilitation And Healthcare Center in Hertford, NC. This PT position is available as full-time or part-time. A relocation bonus of 10K is offered to assist with moving expenses and joining a supportive rehab team.

Responsibilities include comprehensive PT evaluations, development of treatment plans with clear goals, and thorough documentation. Collaboration with the interdisciplinary team is expected to ensure quality resident care.

Qualifikationen

• Graduate of an accredited university with a PT degree (B.S., DPT, MA or MS).
• Current license/registration to practice physical therapy in the state.

Aufgaben

• Provide comprehensive physical therapy evaluations based on physician orders.
• Develop and implement a comprehensive treatment plan with short- and long-term goals.
• Document evaluation findings, treatment plans, progress and discharge summaries.
• Coordinate care with MDs, nursing, and rehab staff; ensure appropriate MD orders for services.
• Document daily participation and progress; maintain timely records and discharge summaries.
• Supervise junior staff or students as needed and co-sign notes as required.
