Shawnee Gardens Healthcare & Rehab Center in Shawnee, KS is seeking a licensed Physical Therapist to develop and manage individualized therapy programs, perform assessments, plan treatments, and communicate progress to the care team. The incumbent will collaborate with an interdisciplinary staff, maintain documentation, and ensure compliance with safety, infection control, and quality standards while supporting patient goals.

Must hold a PT degree and active state license.

Qualifications

• Graduate PT with licensure in the relevant state.
• Knowledge of standards and professional guidelines.
• Strong communication with patients and team.
• Ability to travel as needed for the job.
• Experience with SNF patient populations preferred.

Responsibilities

• Provides direct patient care and evaluates needs with outcomes in mind.
• Designs and implements a care plan with patient and family involvement.
• Reviews and adjusts plans to optimize outcomes.
• Directs support staff to meet patient goals.
• Maintains documentation of evaluations and treatment plans.
• Keeps up to date with professional literature through education.
