Job summary

You will collaborate with care teams, patient families, and nursing staff to ensure continuity of care and optimal outcomes.

Responsibilities include creating therapy plans, documenting progress, and educating families on follow-up care while maintaining a compliant, safe, and organized clinical environment.

Qualifications

• Degree from an accredited Physical Therapy program.
• Current and unrestricted Physical Therapy license in the state of practice.
• Current CPR certification.
• Strong organizational and communication skills.

Responsibilities

• Assess patients, create treatment plans, and deliver therapy to improve function and mobility.
• Educate nursing staff and patient families on follow-up care to support ongoing recovery.
• Document patient progress and ensure compliance with regulatory standards.
• Maintain a safe, clean, and organized work environment.
