Job summary

Shawnee Gardens Healthcare & Rehab Center is seeking a compassionate Certified Occupational Therapy Assistant to join our rehabilitation team. Under the supervision of a licensed Occupational Therapist, you will deliver individualized treatments to promote independence, daily functioning, and quality of life for residents.

You will work with an interdisciplinary team, educate residents and caregivers, monitor progress, and document outcomes to support ongoing therapy needs across facilities.

Qualifications

• Graduate from an accredited Occupational Therapy Assistant program (AOTA approved).
• Active Certified Occupational Therapy Assistant license in the state of employment.
• Maintains continuing education credits as required by state/federal rules.
• Experience in rehabilitation services within SNF or post-acute care preferred.
• Strong communication and collaboration with residents, families and healthcare staff.

Responsibilities

• Provide individualized OT interventions based on treatment plans and patient goals.
• Guide residents through therapeutic exercises to improve strength, mobility, and independence with daily tasks.
• Document progress and changes accurately in the EMR.
• Collaborate with OT, nursing staff, and other disciplines to coordinate care.
• Educate residents and caregivers on adaptive techniques and home exercise programs.
• Assist with dressing, grooming, hygiene and activities of daily living.
• Prepare and organize treatment supplies and equipment; maintain cleanliness and safety.
• Encourage resident participation and motivation throughout the rehab process.
• Participate in interdisciplinary care conferences and share progress updates.
