Job summary

Summit Care LLC in Kansas is seeking a dedicated Physical Therapist to provide comprehensive therapy services to residents and patients. You will evaluate, treat, and discharge plans while supervising assistants and students.

The role requires licensure, strong communication, leadership and adherence to state PT guidelines; you will collaborate with facility staff and families to improve quality of life.

Qualifications

• Accredited Physical Therapy program completion.
• Valid Physical Therapy license to practice in appropriate state(s).
• Effective written and verbal communication skills.
• Effective leadership and customer relations skills.
• Ability to exercise mature judgment with initiative in analyzing and solving problems.

Responsibilities

• Assures execution of doctor’s orders.
• Provides PT screening and evaluation to determine patient status.
• Develops and implements treatment plans with functional outcomes.
• Oversees and participates in maintenance of therapy records.
• Ensures availability of supplies and equipment.
• Maintains appropriate patient schedule per guidelines.
• Documents to maintain quality of medical records and billing accuracy.
• Participates in patient, family, and facility meetings.
• Follows policies including emergency procedures, infection control, and safety.
• Coordinates and/or participates in training programs for facility staff.
• Initiates new program development focused on functional outcomes.
• Participates in continuous quality improvement.
• Patient lifting using biomechanical techniques; capable of lifting 50 lbs.
• Able to walk, bend, stand, and reach consistently during shift.
