Qualifications

• Graduate of an accredited PT program (Bachelor, Master or Doctorate).
• Current license from the state Board of Physical Therapy Examiners.
• Knowledge of SNF standards and patient care protocols.

Responsibilities

• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care for patients based on assessment and includes significant others.
• Conducts therapy practice within defined standards of care.
• Directs the assistance of support staff to achieve patients’ functional goals.
• Communicates effectively with other professional and support staff to achieve positive patient outcomes.
• Follows facility policies, safety standards and quality programs.
• Documents evaluations, treatment goals, and plans; updates documentation regularly.
• Maintains up-to-date knowledge through ongoing education and literature.
