Job summary

TEEMA Solutions Group is seeking a compassionate and licensed Physical Therapist (PT) to evaluate, develop, and implement individualized treatment plans that restore and improve patients' physical function, mobility, and quality of life.

The PT will collaborate with interdisciplinary healthcare teams, provide evidence-based therapeutic interventions, supervise support staff, and ensure high-quality patient care.

Qualifications

• DPT degree required or foreign equivalent (for licenses obtained on/after Jan 2016).
• Active PT license in state of practice.
• Current BLS/CPR certification.
• Two years PT experience preferred.
• Strong clinical assessment, communication, documentation, and patient care skills.

Responsibilities

• Evaluate patients and perform comprehensive physical therapy assessments.
• Develop individualized treatment plans based on patient needs and findings.
• Provide evidence-based therapeutic interventions and protocols.
• Monitor progress and modify treatment plans as needed.
• Create and communicate discharge plans to patients and healthcare team.
• Maintain accurate clinical documentation in a timely manner.
• Collaborate with physicians and interdisciplinary team for optimal outcomes.
• Supervise PT Assistants and support staff as assigned.
• Ensure compliance with policies, safety standards, and regulatory requirements.
• Perform other duties as assigned.
