Job summary

Continuum Therapy Partners is seeking a licensed Physical Therapist for PRN daytime shifts at Claymont Health & Rehabilitation in Uhrichsville, Ohio. You will evaluate and treat patients, develop individualized plans of care, and collaborate with a multidisciplinary team to ensure consistent, high-quality outcomes.

Requirements include a Doctor of Physical Therapy, an active Ohio license, NPTE eligibility, and at least 1 year of PT experience.

Qualifications

• DPT degree required.
• Current, unencumbered Ohio PT license; NPTE must be passed.
• 1 year of PT experience preferred.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on outcomes.
• Educates patients and families on home exercise programs and injury prevention.
