Job summary

Advocate Health is seeking a PRN Physical Therapist for Outpatient and Acute Care opportunities at Atrium Health Rehabilitation Cleveland and Kings Mountain in North Carolina. Qualifications

• State license to practice physical therapy required.

Responsibilities

• Evaluates patients and establishes care plans with modalities, procedures, and exercise programs.
• Gathers data from medical records, physician reports, patient and family, and uses devices as needed.
• Documents evaluations, tests, progress notes, discharge notes and charges.
• Re-evaluates patients as status changes and adjusts goals accordingly.
• Communicates progress to physicians and coordinates schedules and discharge planning.
