Job summary

Advocate Health is seeking a Full-time Outpatient Physical Therapist for Kings Mountain, NC. The role involves evaluating patients, creating and adjusting treatment plans, and coordinating with the care team to support discharge planning in a fast-paced clinical setting.

Qualifications

Bachelor's degree in Physical Therapy required.
Applicable state license to practice Physical Therapy required.
BLS certification required.

Responsibilities

Evaluate patients and establish an individualized care plan including modalities, procedures, exercise programs, therapeutic groups and patient education as warranted by diagnosis and discharge needs.
Gather and interpret data from medical records, physician reports, patient and family with documentation.
Set functional goals with the patient and provide evidence-based intervention with ongoing assessment and plan-of-care adjustments.
Document evaluations, tests, progress notes, treatment notes, education, outcomes, discharge notes and charges.
Communicate evaluation and progress to physicians, referral sources and clinical staff; coordinate schedules and discharge planning.
Provide supervision of PTA's and Rehab Tech/Aide as required for licensure.
