Job summary

People Well Staffing seeks a Physical Therapist to join a collaborative skilled nursing facility team in Los Banos, CA. You will evaluate patients, design individualized plans of care, and provide evidence-based therapeutic exercises to improve mobility, balance, safety, function, and independence.

You will monitor progress, adjust treatments as needed, educate patients and families on home programs, and document care accurately.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active CA Physical Therapist license in good standing, or eligibility for licensure.
• Current BLS/CPR certification as required by the employer.
• Strong clinical reasoning, communication, documentation, and teamwork skills.
• Experience in the applicable setting preferred; qualified clinicians at varying experience levels are encouraged to apply.

Responsibilities

• Evaluate patients and develop individualized plans of care based on functional goals.
• Provide evidence-based therapeutic exercise, mobility, balance, gait, and functional training.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Educate patients, families, and caregivers on safety and home programs.
• Complete accurate and timely clinical documentation.
• Collaborate with physicians, nursing, rehabilitation professionals, and the interdisciplinary team.
