Job summary

Search Station is recruiting a Physical Therapist for a confidential long-term care facility in Los Banos, California to join the Rehabilitation department. The PT will evaluate, plan treatment, educate families, and document care, working closely with the interdisciplinary rehab team and reporting to the Facility Rehab Director.

Requirements include graduation from an approved PT curriculum, active PT license, CPR certification, Medicare knowledge, and LTC experience preferred.

Qualifications

  • Current, active PT license — able to practice unencumbered
  • Good standing with all regulatory agencies and licensing boards
  • CPR Certification, as required for license renewal
  • Working knowledge of Medicare and other payer sources
  • Knowledge of resident rights
  • Long-term care experience preferred
  • PDPM knowledge preferred
  • Knowledge of Group Therapy & Sensory Therapy preferred
  • Knowledge of NetHealth and PointClickCare (PCC) preferred

Responsibilities

  • Initiate evaluations per physician orders and payer authorization/guidelines
  • Develop and provide treatment plans for optimal functional outcomes
  • Educate staff and payers on treatment benefits and outcomes
  • Complete timely documentation including evaluations, progress notes, summaries, and monthly reports
  • Enter key information into EMR and rehab systems
  • Screen individuals who may benefit from therapeutic intervention
  • Collaborate with interdisciplinary rehab team to meet productivity standards
  • Direct and supervise assistants and rehab aides per guidelines
  • Attend CE and provide in-service training to enhance knowledge
  • Submit billing, payroll, and expense documentation promptly
  • Refer to other rehab team members as appropriate
  • Meet or exceed productivity and Group Therapy standards

Compensation: $70 – $75/hour

About the Opportunity

Search Station is recruiting on behalf of a confidential client, a skilled long-term care facility in Los Banos, California seeking a Physical Therapist to join their Rehabilitation department. This role provides and directs all aspects of quality patient care including evaluation, treatment, treatment planning, goal setting, family education, and documentation. The PT serves as a patient and rehabilitation advocate for current and prospective patients, working closely with the interdisciplinary rehab team, and reports to the Facility Rehab Director.

Scope of Work
  • Initiate evaluations per physician orders and payer authorization/guidelines
  • Develop and provide treatment plans consistent with benefit and reimbursement levels for optimal functional outcomes
  • Educate staff, other healthcare professionals, and third-party payers on treatment benefits and functional outcomes
  • Complete timely documentation including evaluations, daily progress notes, summaries, and monthly reports
  • Key information into computer systems, including EMR and rehab systems
  • Screen individuals who may benefit from therapeutic intervention
  • Achieve quality patient care through interdisciplinary co-treatment, protocols, and programs
  • Direct and supervise assistants, rehab aides, and students in accordance with State and Federal guidelines
  • Attend continuing education and provide in-service training to enhance clinical and professional knowledge
  • Submit billing, payroll, and expense documentation in a timely manner
  • Make appropriate referrals to other rehab team members
  • Meet or exceed productivity standard of 87% and Group Therapy standards
Physical Requirements
  • Ability to move intermittently throughout the workday
  • Ability to see and hear (with or without prosthetics) sufficient to meet position requirements
  • Ability to lift up to and over 100 lbs with assistance, according to resident's abilities
  • Ability to sit and walk for extended periods of time
  • Ability to travel to assigned locations within a reasonable time frame
  • Ability to bend, squat, push, stoop, and pull as needed in fulfilling resident care activities
  • Must meet general health requirements including annual TB screening and physical examination
Good to Know
  • This role includes mandated reporter responsibilities regarding suspected elder abuse and access to Protected Health Information (PHI) under HIPAA
  • Environment involves inside work in a 24-hour skilled care setting serving disabled and cognitively impaired residents
  • Both experienced PTs and those newer to long-term care may be considered — LTC experience is preferred, not required
  • Direct supervisory role over assistants, rehab aides, and students included in scope
