Job summary

People Well Staffing is seeking a Physical Therapist to join a collaborative SNF team in Riverbank, CA. You will evaluate patients, develop individualized plans of care, and deliver evidence-based therapies to improve mobility and function.

Strong communication and documentation skills are essential. Requirements include graduation from an accredited PT program, active CA PT license or eligibility, and current BLS/CPR certification.

Qualifications

• Must have CA Physical Therapist license or eligibility.
• Current BLS/CPR certification as required by the employer.
• Strong clinical reasoning, communication, documentation, and teamwork skills.
• Experience in the applicable setting preferred; clinicians at varying levels are encouraged to apply.

Responsibilities

• Evaluate patients and develop individualized plans of care based on functional goals.
• Provide evidence-based therapeutic exercise, mobility, balance, gait, and functional training.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Educate patients, families, and caregivers on safety and home programs.
• Complete accurate and timely clinical documentation.
• Collaborate with physicians, nursing, rehabilitation professionals, and the interdisciplinary team.
