Job summary

People Well Staffing seeks a Physical Therapist to join a collaborative SNF team in Santa Rosa, CA. You will help patients improve mobility, strength, balance, safety, function and independence through individualized, evidence-based care.

Responsibilities include evaluating patients, developing plans of care, providing therapeutic exercise and gait training, monitoring progress, and educating patients and families. CA license and BLS/CPR required; experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active CA Physical Therapist license in good standing, or eligibility for licensures.
• Current BLS/CPR certification as required by the employer.
• Strong clinical reasoning, communication, documentation, and teamwork skills.
• Experience in the applicable setting preferred; qualified clinicians at varying experience levels are encouraged to apply.

Responsibilities

• Evaluate patients and develop individualized plans of care based on functional goals.
• Provide evidence-based therapeutic exercise, mobility, balance, gait, and functional training.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Educate patients, families, and caregivers on safety and home programs.
• Complete accurate and timely clinical documentation.
• Collaborate with physicians, nursing, rehabilitation professionals, and the interdisciplinary team.
