Job summary

WelbeHealth in San Jose, CA is seeking a licensed Physical Therapist to join the interdisciplinary team. You will assess participants, develop care plans with the IDT, and deliver and document restorative therapies with focus on independence and safety. Work includes home visits occasionally and supervision of therapy assistants.

Qualifications

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred.
• California Physical Therapy License, or proof of license eligibility.
• CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Conduct Physical Therapy assessments in the facility and occasionally in the home to define participant care needs, preferences, and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions as agreed upon in participant care plans.
• Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants, providing training and guidance to implement Functional Maintenance Programs (FMP).
• Provide training to staff, participants, and caregivers on use and care of therapeutic equipment.
