Job summary

WelbeHealth is seeking a Physical Therapist to join the interdisciplinary IDT and deliver restorative and rehabilitative services to participants, aiming to maximize independence and safety.

You will conduct assessments, participate in care planning, and provide skilled interventions in the clinic or in participants' homes. WelbeHealth supports work-life balance with generous PTO, holidays, 401(k) match, and ongoing CEUs.

Qualifications

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred.
• Valid California Physical Therapy License or proof of license eligibility.
• CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR/First Aid certification upon hire preferred.
• We seek PTs with outpatient, acute, inpatient, or skilled nursing experience.

Responsibilities

• Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions as agreed upon in participant care plans.
• Provide active and passive exercise, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants, providing training and guidance to implement Functional Maintenance Programs.
• Provide training to staff, participants, and caregivers on the use and care of therapeutic equipment.

The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.

On The Day-to-day, You Will

• Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
• Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment

Job Requirements Include

• Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
• Valid California Physical Therapy License, or proof of license eligibility
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
