Job summary

Post Rehab is seeking a Physical Therapist Assistant (PTA) to join our rehabilitation team in New York, NY. This full-time role offers a great opportunity for a clinician who helps residents improve mobility and regain independence in a skilled nursing and post-acute setting.

You will work under a licensed Physical Therapist, contribute to assessment-based plans, document care accurately, and collaborate with an interdisciplinary team to support residents' mobility and quality of life.

Qualifications

• Active New York Physical Therapist Assistant (PTA) license required.
• Strong communication and teamwork skills.
• Skilled nursing or rehabilitation experience preferred but not required.
• New graduates are encouraged to apply.

Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist.
• Work with residents on strength, balance, gait, mobility, endurance, and functional activities.
• Follow individualized treatment plans based on each resident's goals.
• Monitor progress and communicate changes to the supervising Physical Therapist.
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies.
• Complete accurate and timely clinical documentation.
• Collaborate with PT, OT, SLP, nursing, and other members of the interdisciplinary care team.

What You'll Do
