Job summary

Flagstar Rehab is seeking Physical Therapy Assistants (PTA) for Skilled Nursing Facilities. We connect compassionate PTAs with facilities matching their lifestyle, offering a flexible earning model and remote onboarding.

A NYS PTA license, registration, and BLS are required, with rapid interview processes in place. Hourly pay ranges from $34–$36 for W-2 roles and $38–$40 for 1099, with multiple shift options, benefits, and ongoing support.

Qualifications

• Current NYS PTA license (and registration) is required.
• BLS certification is required.
• Proficient with EMR systems and documentation.
• Must meet all state and federal requirements to work in a nursing facility in the US.
• Malpractice/liability insurance may be required where applicable.

Responsibilities

• Collaborate daily with the PT Supervisor and follow the established Plan of Care for all patients.
• Provide quality therapy services to SNF residents with conditions such as TBI, CVA, stroke, Parkinson’s, MS, cancer, orthopedic surgery.
• Monitor and document patient progress in EMR and care records.

Physical Therapy Assistant Requirements and Responsibilities:

• Current NYS PTA License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
