Job summary

Medi Home Health Agency, a division of Medical Services of America, Inc., is hiring a Full-Time Physical Therapist Assistant to provide in-home therapy across Winston-Salem and surrounding counties. You’ll work under a licensed Physical Therapist to help patients regain strength and independence in their homes.

The role emphasizes compassionate, patient-centered care with clear documentation, strong communication, and teamwork in a home-based setting.

Qualifications

• Completion of an accredited PTA program.
• Active PTA license in North Carolina (required).
• Valid driver’s license and reliable transportation (required).
• Minimum of 1 year of verifiable physical therapy experience (required).
• Home health experience is preferred.

Responsibilities

• Deliver in-home physical therapy services as per plan of care.
• Implement treatment protocols to improve mobility, strength, balance and function.
• Monitor patient progress and communicate updates to supervising PT/Case Manager.
• Maintain accurate, timely documentation according to agency policy and regulations.
• Educate patients and caregivers on exercises and mobility techniques.
• Collaborate with interdisciplinary team to ensure high-quality, coordinated care.

We are currently seeking a Full-Time Physical Therapist Assistant (PTA) to join our Home Health team and provide in-home therapy services across Winston-Salem (Forsyth County) and the surrounding counties.

What You’ll Do:

• Deliver physical therapy services as outlined in the plan of care developed by the supervising Physical Therapist.
• Carry out treatment protocols that support mobility, strength, balance, and functional recovery.
• Monitor patient response to treatment and communicate updates to the PT or Case Manager.
• Maintain accurate, timely documentation in accordance with agency policy and regulatory requirements.
• Assist with educating patients and caregivers on exercises and mobility techniques.
• Collaborate with interdisciplinary team members to ensure coordinated, high-quality care.

What You Bring:

• Completion of an accredited Physical Therapist Assistant program
• Active PTA license in the state of North Carolina (required)
• Valid driver’s license and reliable transportation (required)
• Minimum of 1 year of verifiable physical therapy experience (required)
• Previous home health experience preferred
• Excellent communication, organization, and interpersonal skills
