Continuum Therapy Partners is seeking a Physical Therapist Assistant to join our team at Heritage Manor in Chisholm, MN. This full-time daytime role supports a therapist-led environment with a focus on delivering quality patient care.

The PTA will implement treatment plans under supervision, maintain accurate progress documentation, and educate patients on home exercise programs. A state license and completion of an accredited PTA program are required.

Qualifications

• Associate Degree in Physical Therapist Assistant Program is required.
• Current, unencumbered state Physical Therapist Assistant License is required.
• 1 year of previous experience as a Physical Therapist Assistant preferred.
• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.
