Job summary

Aventura at Walton Hills in Walton Hills, OH, is seeking a licensed Physical Therapist Assistant (PTA) to join our full-time, in-house rehabilitation team.

Work under a licensed Physical Therapist to help residents improve mobility, strength, balance, and independence. Document treatments and progress in Net Health, and collaborate with physicians, nursing, and therapy colleagues while maintaining all standards. Ideal candidates will have SNF/long-term care experience and excellent communication.

Qualifications

• Current Ohio PTA license in good standing.
• SNF/long-term care experience preferred.
• Experience with Net Health preferred.
• Excellent communication, organizational, and teamwork skills.

Responsibilities

• Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Assist residents in improving mobility, strength, balance, endurance, and functional independence.
• Accurately document treatments, resident progress, and outcomes using Net Health.
• Collaborate with physicians, nursing staff, therapy colleagues, and interdisciplinary team members to deliver resident-centered care.
• Maintain compliance with all federal, state, facility, and professional practice standards.
• Educate residents and families on treatment plans, safety techniques, and home exercise programs as appropriate.
