Job summary

Responsibilities include evaluating residents, creating individualized plans, and delivering skilled PT services while documenting progress in Net Health and PCC. Ohio licensure and strong teamwork are required.

Qualifications

• Must hold current Ohio PT license.
• SNF/LTC experience preferred but not required.
