Job summary

Aventura Health Group in Dayton, OH is seeking a Full-Time Physical Therapist to join our SNF/LTC rehab team across two nearby facilities. This split opportunity enables you to provide patient-centered care in a supportive environment.

New graduates are welcome. You will evaluate and treat residents, develop treatment plans, and collaborate with nursing and interdisciplinary teams, while ensuring accurate documentation and regulatory compliance.

Qualifications

• Licensed Physical Therapist (PT) in the state of Ohio.
• SNF/LTC experience preferred, but not required.
• New graduates are welcome to apply.
• Strong communication and clinical skills.
• Positive attitude and team-oriented mindset.

Responsibilities

• Evaluate and treat residents in a SNF/LTC setting.
• Develop and implement individualized therapy treatment plans.
• Help residents improve mobility, strength, balance, and overall function.
• Collaborate with nursing, therapy, and interdisciplinary teams.
• Complete documentation using Net Health and PointClickCare (PCC).
• Maintain compliance with facility and regulatory standards.

Setting: Skilled Nursing / Long-Term Care (SNF/LTC)
