Job summary

Gardens of Mayfield Heights is seeking compassionate Physical Therapist Assistants (PTAs) to join our in-house rehabilitation team in a SNF/LTC setting. We offer Full-Time and PRN opportunities with a supportive, resident-centered approach.

As a PTA, you'll deliver skilled treatments under a licensed PT, develop mobility and functional goals, collaborate with an interdisciplinary team, document in Net Health, and help residents regain independence.

Qualifications

• Ohio PTA license required.
• Graduate of an accredited PTA program.
• Strong communication and teamwork skills.
• Dependable, compassionate, and resident-focused.
• SNF/LTC experience preferred but not required.

Responsibilities

• Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist.
• Carry out individualized treatment plans focused on mobility, strength, balance, safety, and functional independence.
• Work with a geriatric population in a SNF/LTC setting.
• Collaborate with PTs, OTs, COTAs, SLPs, nursing and the rehab team.
• Monitor resident progress and communicate changes to the supervising PT.
• Complete accurate clinical documentation using Net Health.

Skilled Nursing / Long-Term Care (SNF/LTC)

Gardens of Mayfield Heights is looking for compassionate and motivated Physical Therapist Assistants (PTAs) to join our in-house rehabilitation team!

We currently have opportunities available for both Full-Time and PRN PTAs. Whether you're looking to build your career with a full-time position or add flexible PRN hours to your schedule, we would love to hear from you.

What You'll Do

• Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist

• Carry out individualized treatment plans focused on mobility, strength, balance, safety, and functional independence

• Work with a diverse geriatric population in a SNF/LTC setting

• Collaborate with PTs, OTs, COTAs, SLPs, nursing, and other members of the interdisciplinary team

• Monitor resident progress and communicate changes to the supervising PT

• Complete accurate and timely clinical documentation using Net Health

• Provide compassionate, resident-centered care
