Job summary

Aventura at Walton Hills in Walton Hills, OH is hiring a licensed Physical Therapist Assistant (PTA) for a full-time, in-house rehabilitation role within a Skilled Nursing and Long-Term Care setting.

You will collaborate with an interdisciplinary team to improve residents' mobility and independence, while documenting care in Net Health and contributing to a resident-centered care program.

Qualifications

• Current Ohio PTA license in good standing.
• SNF or long-term care experience preferred.
• Familiarity with Net Health documentation.

Responsibilities

• Provide PT treatments under the direction and supervision of a licensed Physical Therapist.
• Assist residents in improving mobility, strength, balance, endurance, and functional independence.
• Document treatments, resident progress, and outcomes using Net Health.
• Collaborate with physicians, nursing staff, therapy colleagues, and interdisciplinary team members to deliver resident-centered care.
• Maintain compliance with all federal, state, facility, and professional practice standards.
• Educate residents and families on treatment plans, safety techniques, and home exercise programs as appropriate.
