Job summary

Aventura at Carriage Inn in Dayton, OH is seeking a Clinical Physical Therapist to join our in-house rehab team. This role offers Full-Time and PRN opportunities and welcomes both experienced clinicians and new graduates.

You will evaluate residents, develop individualized PT plans, help residents improve mobility and independence, collaborate with PTAs, nursing and physicians, document accurately, and ensure compliance with state and federal guidelines using Net Health and PCC.

Qualifications

• Graduate of an accredited Physical Therapy program (US).
• Current Ohio license or eligibility to obtain licensure.
• Strong communication and interpersonal skills.
• New graduates encouraged to apply; SNF/LTC experience not required.

Responsibilities

• Evaluate residents and develop individualized PT treatment plans.
• Provide skilled PT services to SNF/LTC populations.
• Help residents improve strength, mobility, balance and functional independence.
• Establish measurable goals and monitor resident progress.
• Collaborate with PTAs, nursing, physicians, and interdisciplinary team.
• Complete accurate and timely clinical documentation.
• Utilize Net Health and PointClickCare (PCC).
• Participate in discharge planning and resident/family education.

Aventura at Carriage Inn is growing our in-house rehabilitation team!

We are seeking a motivated and compassionate Physical Therapist (PT) to join our team in Dayton, Ohio. Our primary need is for a Full-Time PT, with PRN opportunities also available.

Position Responsibilities

Qualifications
