Job summary

Sigma Health Rehab is seeking a full-time Physical Therapist (PT) to join our skilled nursing facility team in New York. Applicants must be licensed in New York and graduates of an accredited Physical Therapy program; SNF experience is preferred, but new graduates with relevant fieldwork are encouraged to apply.

You will evaluate residents, develop rehab plans, supervise PT assistants, and uphold patient confidentiality while contributing to a multidisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.
• Experience in skilled nursing or geriatric care setting preferred; new graduates with fieldwork experience encouraged to apply.

Responsibilities

• Evaluate residents’ conditions and develop appropriate physical rehabilitation plans.
• Relieve residents’ pain using exercises, hands-on therapy and stretching.
• Assist residents through tasks and monitor their performance.
• Supervise the activities of physical therapy assistants.
• Maintain resident confidentiality and comply with legal and professional requirements.
• Collaborate with the healthcare team to provide coordinated care.
