Job summary

Sigma Health Rehab is seeking a full-time Physical Therapist (PT) to work in a Skilled Nursing Facility. You will evaluate residents, create rehabilitation plans, manage pain, and supervise PT assistants.

Requirements include graduation from an accredited PT program and a NY license. SNF/geriatric experience is preferred; new grads are encouraged to apply. A $5,000 sign-on bonus and competitive benefits are offered.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.
• Experience in a skilled nursing or geriatric care setting preferred; new grads welcome with relevant fieldwork.

Responsibilities

• Evaluate residents’ conditions and develop appropriate rehabilitation plans.
• Relieve residents’ pain using exercises, hands-on therapy and stretching.
• Assist residents with activities and monitor their performance.
• Supervise the activities of physical therapy assistants.
• Maintain resident confidentiality and comply with legal requirements.
• Collaborate with healthcare team to provide cohesive care.
