Sigma Health Rehab is seeking a full-time Physical Therapist (PT) to join our skilled nursing facility team in New York. The role focuses on evaluating residents, developing rehabilitation plans, and applying hands-on therapies to relieve pain and restore function.

Qualifications

• Graduate of an accredited Physical Therapy Program.
• Current New York State PT License.
• Experience in a skilled nursing or geriatric care setting is preferred, but new graduates with relevant fieldwork experience are encouraged to apply.

Responsibilities

• Evaluate residents’ conditions and develop appropriate physical rehabilitation plans.
• Relieve residents’ pain using exercises, hands-on therapy and stretching.
• Help residents through their tasks and monitor their performance.
• Supervise the activities of physical therapy assistants.
• Maintain resident confidence and protect the facility by keeping information confidential.
• Comply with federal, state, and local legal and professional requirements.
• Maintain a cooperative relationship among healthcare team members.
