Job summary

Nexushm in Chicago, IL is seeking a committed Physical Therapist to develop individualized treatment plans for residents to regain independence and safely participate in daily activities. Ideal candidates hold an Illinois license and have SNF/long-term care experience, with up-to-date knowledge of treatment practices and PCC documentation.

Qualifications

• Active Illinois license in Physical Therapy is required.
• SNF/long-term care experience preferred.
• Up-to-date knowledge of current PT treatment practices.
• Familiarity with PCC Electronic Documentation System preferred.

Responsibilities

• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
