Watsontown Nursing and Rehab Center in Watsontown, PA is seeking a dedicated Full-time Physical Therapist (PT) to join its in-house rehabilitation team. The role focuses on person-centered care, individualized treatment plans, and collaborative practice with the interdisciplinary team.

Watsontown Nursing and Rehab Center has an exciting opportunity for a Full-time Physical Therapist (PT) in Watsontown, PA! We are looking for a dedicated PT to join our in-house rehabilitation team of outstanding professionals who provide person-centered care to our residents and patients. Our goal is to help each individual achieve meaningful outcomes while maintaining the highest standards of quality and compassion. At Watsontown Nursing and Rehab Center, our clinical teams are committed to providing individualized care with compassion, collaboration, and excellence. Our staff works together to develop customized treatment programs and best practices designed to meet the unique needs of each resident and exceed industry standards.

Physical Therapist Responsibilities

• Consult patients to understand their individual needs, concerns, and rehabilitation goals.
• Perform comprehensive patient evaluations and assess and interpret evaluation findings and test results.
• Develop individualized treatment plans using a variety of evidence-based physical therapy techniques and interventions.
• Create personalized rehabilitation programs designed to help patients achieve their functional goals and maximize independence.
• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, increase mobility, and promote healing.
• Ensure all treatment is provided in accordance with the established plan of care.
• Monitor patient responses to treatment interventions and modify treatment approaches as clinically appropriate in collaboration with the interdisciplinary team (IDT).
• Provide clinical support, education, and guidance to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to promote coordinated, high-quality patient care.
• Ensure all documentation is completed accurately, thoroughly, and in a timely manner in accordance with company policies and applicable state and federal regulations.
• Comply with all applicable rules, regulations, policies, procedures, and professional standards.
• Attend required meetings and participate in other duties as designated by the Director of Rehabilitation.

Physical Therapist Skills & Qualifications

• Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.
• Current knowledge of physical therapy treatment practices, techniques, and interventions.
• Familiarity with the Net Health electronic documentation system is preferred.
• Strong clinical, communication, and interpersonal skills.
• Ability to effectively work with patients and residents with a variety of personalities, needs, and abilities.
• Strong organizational skills and attention to detail.
• Ability to collaborate effectively with patients, families, caregivers, and members of the interdisciplinary team.
• Commitment to providing compassionate, patient-centered care.

Physical Therapist Requirements

• Active and valid Physical Therapy license in PA.
• Must be in good standing with all applicable regulatory agencies and licensing boards.
